Coquitlam RCMP are asking for the public's help in finding a patient who's been missing from the Colony Farm Forensic Psychiatric Hospital for two weeks.

Matthew Mikolajczyk, 26, left the hospital without permission on June 16 and hasn't been heard from since.

Because he hasn't had access to regular treatment since then he's considered to be a risk to himself and the public. There is a Canada-wide warrant for his arrest.

Mikolajczyk is described as an aboriginal male with dark hair and brown eyes. He is about six feet tall and weighs 170 pounds. He has a seven-inch scar on his right arm, and was last seen with a full beard, wearing a red t-shirt and white pants.

Anyone with information as to Mikolajczyk's whereabouts should call Coquitlam RCMP at 604-945-1550; if you see him do not approach him and call 911 immediately.
